First Aid – Action Plans and Medication
For us to give your students the best care possible, we encourage families with students who require regular medical care at the College, to contact their local GP during the school holidays for an up-to-date Action Plan and medication. This may be for Asthma, Allergies, Anaphylaxis and Diabetes.
If your student requires any medication they must bring it along with them to school each day, with the pharmacy label noting your doctor’s instructions, in its original packaging.
Along with the signed Action Plan from your doctor, a Request to Administer Medication form filled out by the student’s parent/guardian.
These forms can also be obtained from Administration and First Aid.
Please feel free to call us on 03 5997 1444 or email us at koo.wee.rup.sc@education.vic.gov.au.
Thank you for assisting us in keeping your students safe!
